All on six implants offer a breakthrough for a natural-looking, high stability, and functional restoration compared to the conventional removable denture.

What is all on six implants?

All on six implants technique is a surgical solution for full mouth rehabilitation, where six implants are embedded in the jaw bone to act as a base for a fixed prosthesis.

There are three main components in the all on six implants: implants, abutments, and prosthetic restoration.

The six screws or implants are titanium made, which is a highly biocompatible material with the jaw bone and soft tissue around. Our expert surgeon professionally plans these implants to be placed at different places and angles to allow for equal force distribution. They act as an anchor for the restoration.

Titanium abutments represent the connection between the restoration and the implants embedded in the jaw bone.

Prosthetic restoration comprises a titanium framework covered by a sturdy acrylic resin layer. The framework supports the artificial teeth constructed from porcelain or zirconia to give a natural smile look.

Who is an ideal candidate for an all on six implant?

  1. Completely edentulous patients.
  2. Patients looking for better stability of their complete dentures.
  3. Patients with no medical conditions contraindicating surgery (bleeding tendency, diabetes)
  4. Patients with sufficient bone height and density

All on six implant procedure at Dulwich Dental Office

1. Consultation and assessment:

Our expert team, led by Dr. Javier Perez, will conduct a series of examinations assessing all the parameters related to the implant site to decide the most suitable treatment plan.

As a start, a thorough intra-oral examination will take place. This step is for the assessment if any additional dental procedure is needed, such as extraction of any remaining roots (if existing).

A cone beam CT is mandatory to collect all the required data for careful treatment planning about bone density, width, and approximation to any vital structures.

The collected data will help choose the most appropriate implant location, direction, length, and width. Also, to evaluate if an additional step of bone graft procedure is needed.

This meticulous data collection procedure and treatment planning ensure implant success.

Our team will discuss the final treatment plan with you to share the procedure steps and the expected outcome. Also, detailed post-operative instructions will be provided and explained to you.

2. Surgical procedure: 

The surgery is performed under local anesthetics. The procedure takes about 2-4 hours.

The six implants are secured in their places, followed by a temporary resin denture placement to aid during healing until the final denture construction. But in some cases, the final restoration is fabricated and inserted on the same day.

Follow the post-operative instructions carefully during the healing period to ensure successful implant healing and integration with the surrounding bone.

3. Final prosthesis: 

The final prosthesis insertion may take up to 6 months until complete healing and osteointegration around the implants. The Final prosthesis is screwed to the implant abutments to act as a fixed restoration that you cannot remove by yourself.

Post-operative care:

1. Within the first six months, avoid hard food until the complete healing of the implants.

2. Quit smoking and avoid alcohol and acidic beverages.

3. Care and maintenance: follow the suggested oral hygiene instructions on how to clean your restoration to keep a radiant smile for a long time and avoid gums inflammation underneath the restoration. These include:

  • Regular brushing and flossing are essential in your daily routine. Brush twice daily, mainly after meals, and floss at least once.
  • Water picks are advantageous to help remove any remaining food particles.
  • The use of antiseptic mouthwashes.
  • Use a soft toothbrush and avoid abrasive toothpaste.

4. Schedule follow-up visits every six months to monitor the condition of your implants and restoration and address any problems as early as possible.

Pros of all on six implants:

  1. Create more strength and better support for the final restoration than all-on-4 implants and conventional removable dentures.
  2. Natural looking.
  3. Stability: no worries that the denture may become loose or fall, which causes embarrassment.
  4. Permanent solution for completely edentulous patients who experience nausea with conventional complete dentures.
  5. Less costly than the conventional 8-implant technique.
  6. Easy to clean and maintain.
  7. Durable and robust
  8. Eat what you want: no diet limitations as with complete dentures.
  9. Prevent bone loss, unlike conventional complete dentures that cause bone resorption.

Pros of all on six implants:

  1. Costlier than all-on-4 implants.
  2. As with all surgical procedures, it may involve some risks: bleeding, nerve damage, swelling, and pain.
  3. Diet limitation for the first six months after surgery.
  4. It may take up to six months to deliver the final restoration until complete healing occurs.
  5. It may require an additional step of bone graft if there is no sufficient bone height and width.


The surgery and the temporary prosthesis are completed in one day, while the final prosthesis may take up to six months.

During the first three to six months, it is essential to avoid hard food until complete healing. There are no additional limitations.

It is possible to experience some pain, swelling, and bruising after surgery. Use the prescribed painkillers. These symptoms will disappear within a few days.

Mainly brush twice and use dental floss once daily after meals. A water pick is valuable to remove any remaining food particles under the prosthesis that may lead to inflammation. Keep away from abrasive toothpaste and hard toothbrush.

Dr Javier has completed a Masters in Implant Dentistry MClinDent (Implant) in London over three years and was awarded a diploma in Oral Surgery from the Royal College of Surgeons of England, one of the most prestigious colleges in UK.
Dr Perez has trained in Oral Surgery at the Maxillofacial Unit in the Queen Victoria Hospital NHS Foundation Trust in South England. He is at the forefront of dental implants and oral surgery regularly attending National and International conferences to develop his skills and learn about the latest trends in Implantology.
